No — fibromyalgia is not currently classified as an autoimmune disease, though this is a common and understandable question.
Autoimmune diseases occur when the immune system mistakenly attacks the body's own healthy tissue, as in lupus or rheumatoid arthritis. Fibromyalgia is primarily understood as a central sensitization syndrome — a condition in which the brain and spinal cord amplify pain signals, lower the threshold for pain, and dysregulate the nervous system's response to sensation, stress, and environment.
That said, the science is evolving. Groundbreaking research — including a 2021 study published in the Journal of Clinical Investigation by Goebel et al. — demonstrated that IgG antibodies from fibromyalgia patients, when transferred to mice, produced fibromyalgia-like symptoms, suggesting an immune-mediated component may be involved in at least a subset of cases. This does not make fibromyalgia autoimmune by current classification, but it does signal that the immune system's role is more complex than previously understood.
Fibromyalgia also commonly coexists with autoimmune conditions such as lupus, rheumatoid arthritis, and Sjögren's syndrome — which is part of why the distinction can feel blurry.
